Athletic Department Physicals

Chapelle Gym
Athletes are to be here at 230pm
Histeppers are to be here at 300pm
Cheerleaders are to be here at 330pm
Forms will be available in my office and on the Athletics page of the ACHS website
no later than Monday, May 9
Cost is $20 made payable to ACHS
Please make sure all students are aware of the date and start time for each group
Athletes enrolled in advanced PE for next year MUST have a completed physical on file in order to remain in the class
